Key Responsibilities Provide Disease Management services to insurance beneficiaries Collaboratively assess, plan, implement, coordinate, monitor, and evaluate beneficiaries' health and psychosocial needs Develop care plans in collaboration with providers and assess effectiveness to improve health outcomes Required Qualifications Current, unrestricted RN licensure in the state of residence Ability to pass a Department of Defense background investigation Capability to obtain multi-state licensure if residing in a NCL state Minimum of three years clinical experience Experience in case management, care coordination, or discharge planning in relevant healthcare settings
