Key Responsibilities Engage and educate caregivers via phone and digital technology to improve their wellbeing Manage a caseload and assess caregiver and care recipient needs to create care plan goals Document coaching activities and collaborate with stakeholders to identify problems and solutions Required Qualifications and Education 4+ years of experience in care planning or case management Experience working with the elderly or adults with disabilities Solid understanding of care planning and case management principles Proven ability to access community resources and navigate mobile applications Must be available to work 10-7 EST/9-6 CST, Monday to Friday
