Key Responsibilities: Provide behavioral health support to patients and families navigating kidney disease Coordinate resources and connect patients with community support services Assist with advance care planning and educate on palliative and hospice care options Qualifications: Master's degree in Social Work from an accredited institution State licensure as a Licensed Clinical Social Worker (LCSW) required 2-4 years of clinical experience, preferably in healthcare settings 2 years of renal experience preferred Bilingual preferred
