Key Responsibilities: Collaborate with Care Team to support patient and provider engagement for medication therapy management Conduct Comprehensive Medication Reviews and Medication Reconciliation Post-Discharge for high-risk patients Engage providers with actionable drug-therapy recommendations and document all interventions accurately Required Qualifications: Graduate of an accredited School of Pharmacy with a Doctor of Pharmacy (Pharm.D.) degree Current Pharmacist licensure in good standing in the state of residence 3+ years of direct patient care experience in Medication Therapy Management 1+ year of experience with transitions of care and medication reconciliation Ability to work flexible hours, including evenings, and must reside in the Eastern Time Zone
