A company is looking for a Care Coach (RN, LPN, LVN) to engage members in health and wellness coaching. Key Responsibilities Engage and maintain members in active participation in the health program and coordinate member care Utilize motivational interviewing and skill-building techniques to support members' health goals Collaborate with care team members to improve quality of care and enhance program development Required Qualifications Preferred Associate's or Bachelor's degree in Nursing, or equivalent education/experience Extensive experience in a healthcare consumer-facing environment and implementing care plans Experience working with the Serious Mental Illness population Background or interest in mental health and behavioral health is preferred Self-driven with strong organizational and decision-making skills
