Mississippi Licensed Care Manager

A company is looking for a Care Manager NICU who must be LSW and reside in Mississippi, working from home. Key Responsibilities Engage members and their support systems through strength-based and trauma-informed assessments Develop and implement individualized care plans in collaboration with an interdisciplinary care team Coordinate care and services, ensuring effective communication and collaboration with members and providers Required Qualifications, Training, and Education Nursing degree from an accredited program or Bachelor's degree in a healthcare field, or equivalent experience A minimum of three years of experience in nursing, social work, counseling, or related healthcare fields Preferred experience in Medicaid and/or Medicare managed care Current unrestricted clinical license in the state of practice as a Registered Nurse, Social Worker, or Professional Clinical Counselor Case Management Certification is highly preferred
