A company is looking for a Telephonic Care Navigator. Key Responsibilities: Conducts telephonic outreach to members and assists with scheduling provider appointments Educates members on care gaps and connects them with community resources Maintains adherence to metrics and ensures confidentiality of health information Required Qualifications: High School Diploma / GED or equivalent work experience At least 1 year of call center or telephonic customer service experience 1 year of professional experience in an office setting using phone and computer 1 year of experience in healthcare, insurance, or community outreach Ability to work full-time and attend all training sessions without time off
