A company is looking for a Telephonic Behavioral Health Caseworker. Key Responsibilities Manage and coordinate care for Members with behavioral health conditions and complex needs Support Members during transitions of care and ensure medication reconciliation Conduct telephonic visits for education and assessment, maintaining accurate documentation Required Qualifications Bachelor's Degree in Human Services or related field; Master's preferred 2 years of experience working with Serious Mental Illness (SMI) members 2 years of social services experience engaging with clients and assessing needs Proficiency in MS Office and electronic medical record documentation Ability to manage a caseload of up to 60 Members and support care transitions
