Key Responsibilities Build empathetic relationships with patients and families while providing education on diagnosis and treatment options Conduct advance care planning discussions to identify patient needs and develop collaborative care plans Coordinate care across multiple providers and ensure accurate documentation of patient care Required Qualifications Current RN License required; BSN preferred 1+ years of utilization management experience preferred 3+ years of clinical practice in an ICU, hospice, or palliative care role Experience in goals of care discussions and end-of-life care decisions required Intermediate skills with MS Office Suite and ability to work effectively in a remote environment
