Key Responsibilities Evaluate member needs through post-discharge assessments and medication reconciliation Develop care/service plans and collaborate with healthcare teams to support member transitions Facilitate services and provide education to promote wellness and improve overall quality of care Required Qualifications, Training, and Education Master's degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing 2 - 4 years of related experience Licensure as LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN is required
