A company is looking for a Clinical Navigation, Family Nurse Practitioner (Per Diem). Key Responsibilities: Assess patients' needs, gather clinical history, and provide guidance to navigate healthcare resources Demonstrate clinical acumen across various services and maintain quality of care standards Evaluate and manage patients with acute, chronic, and mental health conditions while collaborating on efficiency projects Required Qualifications: Licensed NP/PA provider in good standing with state board Board certification maintained At least 3 years of NP/PA experience in a clinical setting Comfortable providing care to all age groups Must be in PST or MST time zone
