Arkansas Community Care Coordinator

A company is looking for a Community Based Care Coordinator to engage with members and assist in their health management. Key Responsibilities Engage members to complete health and psychosocial surveys while considering their cultural and linguistic needs Develop individualized care plans in collaboration with the interdisciplinary care team and manage barriers to care plan goals Coordinate with community-based case managers and service providers to ensure effective care transitions and avoid service duplication Required Qualifications Diploma, Associate's, or Bachelor's Degree in a healthcare-related field or equivalent relevant work experience Minimum of three years of experience in a healthcare field such as discharge planning, case management, or care coordination Ability to work remotely in Arkansas and availability to work 40 hours a week until 12/31/25 Must have personal equipment that meets specified technical requirements
