Care Management Coordinator II
A company is looking for a Care Management Support Coordinator II.
Key Responsibilities
Provides outreach to members to support care plan next steps and connect them with health plan resources
Serves as front-line support for member and provider inquiries, explaining care plan procedures and protocols
Documents and maintains non-clinical member records in accordance with state and regulatory requirements
Required Qualifications
High School diploma or GED
1 - 2 years of related experience, preferably in Healthcare Call Center, Customer Service, or Claims
Knowledge of assigned health plan activities and resources
Ability to connect members with community resources addressing Social Determinants of Health (SDOH)
For Florida-Sunshine Health Plan, all interactions with members must be conducted telephonically
