A company is looking for a Case Management Coordinator. Key Responsibilities Manage a caseload of 20-25 cases focusing on care coordination and continuity-of-care Utilize various sources of information to implement case management care plans Maintain documentation of care plan progress and submit reports every 4 weeks Required Qualifications High school diploma or GED 2-3 years of experience in customer service, claims, and/or triage Coordination of care experience in an insurance or hospital setting Computer proficiency and working knowledge of Microsoft Office Suite and Outlook Knowledge of medical claims and ICD-10, CPT, HCPCS coding preferred
