Case Management Coordinator
A company is looking for a Case Management Coordinator.
Key Responsibilities
Conduct comprehensive evaluations of referred members' needs and recommend case resolution approaches
Coordinate and implement care plan activities while monitoring progress
Consult with multidisciplinary teams to overcome barriers and escalate quality of care issues as needed
Required Qualifications
Bachelor's degree in behavioral health or human services, or a non-licensed master-level clinician
2 years of experience in behavioral health, social services, or a related field
Confidence in working independently and using virtual collaboration tools
Proficiency with MS Word, Excel, Outlook, and PowerPoint
Experience in case management and discharge planning
