A company is looking for a Hospital Coordinator & Case Manager for the Readmissions Reduction Program. Key Responsibilities Oversee and coordinate the Readmissions Reduction Program, ensuring effective communication with hospital staff Serve as a case manager for clients with mental illness and/or substance use disorders, providing support and guidance Manage data entry and documentation related to patient intake and care coordination Required Qualifications Bachelor's Degree in psychology, social work, or a related behavioral health discipline 2-4 years of relevant experience in behavioral health, including case management and crisis intervention Knowledge of community resources and benefits related to mental health and substance use disorders Proficiency in Google Suite and Microsoft Office software Ability to work in a fast-paced environment and manage multiple tasks effectively
