A company is looking for a MSW Inpatient Case Manager - Contingent. Key Responsibilities Provide therapeutic intervention and social work services to patients and families Conduct patient assessments and develop care plans while coordinating with providers Serve as a patient advocate to facilitate continuity of care in the community Required Qualifications Master's degree in Social Work Unrestricted LMSW licensure in the state of Michigan Minimum of three (3) years clinical experience Certification in Case Management (CCM) or Accredited Case Manager (ACM) preferred Self-motivated and able to work in a fast-paced environment
