A company is looking for a Clinical Navigation, Family Nurse Practitioner (Per Diem). Key Responsibilities: Assess patients' needs, gather clinical history, and provide clinical guidance while navigating them to appropriate resources Deliver high-quality clinical services across various Included Health service lines and monitor care quality Evaluate and manage patients with acute, chronic, and mental health conditions, collaborating on projects to improve efficiency and member experience Required Qualifications: Licensed NP/PA provider in good standing with state board Maintains board certification At least 3 years of NP/PA experience in a clinical setting Comfortable providing care to all age groups Must be in PST or MST time zone
