A company is looking for a Behavioral Health Case Manager.
Key Responsibilities
Coordinate and support mental health care for high acuity patients and their families
Develop, monitor, and evaluate treatment plans while facilitating transitions to community-based services
Collaborate with a multidisciplinary team to ensure efficient patient care and enhance satisfaction
Required Qualifications
Experience in clinical case management, particularly with high acuity patients
Understanding of mental health needs and social services in the community
Ability to assess crisis situations and respond appropriately
Certification in CANS/ANSA or willingness to obtain
Adherence to HIPAA regulations and professional ethics
