North Carolina I/DD Coordinator
A company is looking for an IDD In Reach & Transition Coordinator (Remote NC).
Key Responsibilities
Identify consumers/enrollees for potential local community placement from State and Local Hospitals and facilities
Assist in the transition process by matching consumers/enrollees with available housing and coordinating support services
Document in-reach and transition progress accurately for each individual, ensuring compliance with local, state, and federal requirements
Required Qualifications and Education
Bachelor's degree in public service administration, psychology, sociology, social work, or a related field with four years of relevant experience, or a Master's degree with two years of supervised clinical experience
Experience in case management, assessment and referral, supportive counseling, intervention, psycho-social therapy, and treatment planning
Experience working with children, adolescents, and adults with mental health and intellectual/developmental disabilities
Ability to establish respectful relationships with clients and their families
Residency in North Carolina or within 40 miles of the North Carolina border is required
