A company is looking for a Lead Practice Liaison, Chronic Therapies. Key Responsibilities Coordinate and communicate with Chronic providers to manage referral flow and enhance patient care Identify and resolve referral flow obstacles, collaborating with sales and operations teams Develop strong relationships with key accounts to foster improved referral processes and outcomes Required Qualifications High School Diploma with 3 years of customer service/account management experience or an Associate's Degree, LPN, or Pharm Tech license Proven experience in building strong internal relationships with Operations/Sales Teams Expertise in managing the referral process and resolving referral flow obstacles Knowledge of the requirements for referrals in various therapeutic areas Proficient in using relevant platforms for referral management and data entry
