Key Responsibilities: Assess patients' needs, gather clinical history, and provide guidance to navigate them to appropriate services and resources Deliver high-quality clinical services across various Included Health service lines, ensuring patient satisfaction and clinical effectiveness Evaluate and manage patients with acute, chronic, and mental health conditions while collaborating on projects to enhance member experience Required Qualifications: Licensed NP/PA provider in good standing with state board Maintains board certification At least 3 years of NP/PA experience in a clinical setting Comfortable providing care to all age groups Must be in PST or MST time zone
