Indiana Licensed Care Manager
A company is looking for a Care Manager for a Suicide Prevention Program.
Key Responsibilities
Assist with outreach, engagement, and continuous support for members, providing regular check-ins and personalized care
Develop and coordinate community resources to address medical, behavioral, violence prevention, and social service needs
Assess risk, perform crisis interventions, and ensure safety through tailored safety plans
Required Qualifications
Bachelor's degree in psychology, social work, or a related behavioral health discipline from an accredited institution
Master's degree in a related field is preferred
At least 2 years of experience in a related role, preferably with at-risk adolescents and young adults
Crisis intervention training or similar certification is required
Knowledge of local resources, mental health, substance abuse disorders, and social issues
