A company is looking for a Case Manager for their Hospital Readmission Reduction Program. Key Responsibilities Manage a caseload of Members with mental illness and/or substance use disorders, ensuring successful discharge and community transition Provide crisis intervention, coordinate care, and educate Members and their families about available resources Maintain accurate documentation and collaborate with treatment teams to assess and address Members' needs Required Qualifications 2-4 years of relevant experience in the behavioral health field Bachelor's Degree in psychology, social work, or a related behavioral health discipline Knowledge of community resources and social determinants of health Familiarity with benefits such as SSDI, SSI, Medicaid, and housing vouchers Proficient in Google Suite and Microsoft Office software
