Licensed Mental Health Coordinator

A company is looking for a Population Health Coordinator - Mental Health. Key Responsibilities Manage data and registries for special populations, facilitating care coordination and quality improvement initiatives Provide direct member care through outreach, assessments, and education while supporting complex cases Assist in pilot projects and collaborate across various disciplines to enhance care delivery methods Required Qualifications Registered Nurse (RN) or Licensed Practical Nurse (LPN) with specific training for medical focus; licensed clinical social worker for mental health focus; or certified social worker/peer for social care focus At least 5 years of experience in relevant fields such as addiction medicine, mental health, or case management Bachelor's degree in Nursing preferred for medical focus roles Proficiency in EMRs, Microsoft Office/Google Suite, and data analysis Access to reliable internet and telephone services for remote work roles
