Ohio Licensed Care Manager RN

A company is looking for a MBB Care Manager for Ohio, Indiana, and/or Georgia based RN's. Key Responsibilities Engage members and their support systems through assessments and care planning Facilitate inter-disciplinary care team meetings and coordinate care efforts Educate members on treatment options, community resources, and monitor care plan progress Required Qualifications, Training, and Education Nursing degree from an accredited program or Bachelor's degree in a health care field A minimum of three years of experience in nursing, social work, or related fields Three years of maternity experience is required Current unrestricted clinical license as a Registered Nurse, Social Worker, or Professional Clinical Counselor Case Management Certification is highly preferred
