A company is looking for a Care Coordinator - Determination of Needs. Key Responsibilities Educate families about the Ontario Autism Program and available service options Maintain accurate applicant information in the database Conduct determination of needs interviews to assess support requirements for children Required Qualifications Bachelor's Degree with a minimum of 3 years of experience in human services or a related field Experience working with families of children with Autism Spectrum Disorder or developmental disabilities Knowledge of community-based resources and the developmental services sector Fluency in written and oral French, Mandarin, or Cantonese is preferred Must pass a police reference check and be available to work evenings and weekends
