Patient Support Manager
A company is looking for a Patient Support Case Manager to join their organization.
Key Responsibilities
Act as the primary point of contact for patients, caregivers, and healthcare providers throughout the patient journey
Identify barriers to therapy and assist patients in navigating these challenges to maintain prescribed treatment
Provide educational resources and support via inbound and outbound calls to ensure a successful patient experience
Required Qualifications, Training, and Education
Minimum of 5 years of pharmacy experience required
Associate degree or equivalent program in pharmacy or healthcare preferred
Desired certifications include PACS, CHES, or CCM in healthcare or social science
Minimum of 2 years of patient support program experience preferred
Pharmacy Technician certification is desired
