Pennsylvania Licensed Health Care Manager

A company is looking for a Health Care Manager - Special Needs Plan (SNP). Key Responsibilities Ensure continuity and coordination of care for members with chronic conditions and complex health needs Develop and coordinate individualized treatment plans and evaluate their effectiveness Monitor and evaluate treatment plans, making necessary adjustments to optimize care services Qualifications Master's degree in a human service field or Licensed Pennsylvania RN with 6-8 years of clinical experience Five years of experience in clinical, care coordination, and/or case management General knowledge of best practices in health care, especially for special needs populations Computer proficiency required; experience with Microsoft Office preferred Registered Nurse (RN) or Social Services Licensure (LSW, LCSW, LPC) required
