Tennessee Licensed Patient Care Advocate
A company is looking for a Patient Care Advocate to enhance member and provider experiences through outreach and education.
Key Responsibilities
Act as a liaison between members, families, and providers to address care gaps
Conduct telephonic outreach and face-to-face education regarding preventive services and care barriers
Schedule appointments and arrange transportation for members to access necessary services
Required Qualifications
Bachelor's Degree in Healthcare, Public Health, Nursing, Psychology, Social Work, Health Administration, or related field, or equivalent work experience
A total of 4 years of experience in direct patient care, social work, quality improvement, or health coaching, preferably in a managed care environment
At least 2 years of experience in direct patient care, social work, quality improvement, or health coaching
Preferred certifications include Licensed Practical Nurse (LPN), Licensed Master Social Work (LMSW), Certified Social Worker (C-SW), Licensed Social Worker (LSW), or Licensed Registered Nurse (RN)
