A company is looking for a Regional Hospice Bereavement Coordinator. Key Responsibilities Assess bereavement services with care center leadership to meet community and patient needs Develop individualized patient bereavement care plans and conduct post-death bereavement risk assessments Provide support and outreach to grieving individuals and collaborate on program improvement initiatives Required Qualifications Bachelor's degree in social work, psychology, counseling, or related field One year of experience in family and/or individual counseling with training in grief and bereavement Knowledge of grief and bereavement counseling and the needs of terminally ill patients and caregivers Familiarity with general computer use and online HR services Self-management of online training and services
