Key Responsibilities Conduct telephonic case management for health plan members Support care coordination and develop Plans of Care Focus on Special Needs Plans (SNPs) for Medicare Advantage members Required Qualifications Active Compact State RN License Minimum 2 years of Case Management experience within a health plan Experience with Special Needs Plans (SNP) Proficiency in Microsoft Office Bilingual (English/Spanish) preferred; CCM certification is a plus
