Key Responsibilities Conduct outreach to members to complete surveys addressing Social Determinants of Health (SDoH) barriers Coordinate care by reaching out to members and clinical providers to obtain medical records and ensure preventive and chronic disease management services Search for relevant HEDIS records in databases, review medical records, and ensure accurate documentation for compliance Required Qualifications High School Diploma or GED At least two HEDIS seasons of experience Experience in making outreach calls to educate members and providers Medical knowledge and terminology proficiency Proficiency in Microsoft Word, Excel, Outlook, and Adobe Reader
