Massachusetts Licensed RN Care Manager

A company is looking for an RN Care Manager. Key Responsibilities Conduct comprehensive clinical assessments and develop patient-centered care plans Coordinate all aspects of patient care across the continuum and advocate for patient needs Engage with patients and families to establish trusting relationships and promote self-management goals Required Qualifications Massachusetts RN Licensure or current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC) Three years of clinical experience in an acute care setting Basic Life Support Certification or obtain within three months Preferred: Bachelor's degree and five years of experience in acute, subacute, or home care Preferred: Case Management certification and bilingual proficiency
