A company is looking for a SNBC Wellness Coordinator. Key Responsibilities Make outreach calls and send letters for care coordination Manage a caseload of members, tracking tasks and completing assessments Assist members with information about care coordination services and community resources Required Qualifications High School Diploma / GED or equivalent work experience Must be 18 years of age or older Ability to work in-person up to three days per week as needed Literacy with computer databases and knowledge of community resources Positive attitude towards members with significant mental health disabilities
