Key Responsibilities Evaluate new and current NHTD Waiver participants and make recommendations for level of care changes Conduct informational meetings and community outreach to disseminate information about waiver services Monitor waiver services and expenditures, review and approve service plans, and provide training and technical assistance Required Qualifications Graduate of an accredited Registered Nurse program Current licensure as an RN in the state of New York Two years of satisfactory professional experience working with individuals with disabilities and/or seniors in a community setting Clean, valid NYS driver's license (no major violations within the last 5 years) BS or AS in Nursing
