A company is looking for a Care Manager. Key Responsibilities Develops and maintains positive relationships with members, families, and care teams to enhance health and quality of life outcomes Conducts comprehensive health assessments and develops, implements, and evaluates care plans for members Coordinates care team meetings and ensures effective communication among all stakeholders involved in the care process Required Qualifications Two years of experience as a Care Manager, Case Manager, or Care Coordinator preferred Must meet all agency and state requirements for pre-employment A license or certification in a human service profession is required A Master's degree in a human service field with one year of relevant experience, or a Bachelor's degree in a human service field with two years of relevant experience is preferred Experience with individuals with intellectual and developmental disabilities (IDD) is necessary
