A company is looking for a Case Manager to support members in achieving healthier lifestyles through integrated case management. Key Responsibilities Engage with target members to build relationships and facilitate open discussions on sensitive health topics Conduct holistic health assessments to identify member needs and develop personalized case management plans Monitor and evaluate the effectiveness of case management plans, ensuring coordination among healthcare providers Required Qualifications Current Registered Nurse (RN) license allowing independent assessments Minimum of 3 years of direct patient or clinical care experience post-licensure High-speed internet connection (20 Mbps or higher) Ability to obtain Certified Case Manager (CCM) certification within 24 months if not already certified Valid Washington state or MSL/Compact license meeting state credentialing requirements
