A company is looking for an RN Case Manager in California. Key Responsibilities Coordinate all aspects of a patient's care and serve as a patient advocate Conduct ongoing care coordination and needs assessments for complex patients Participate in interdisciplinary care team meetings and quality improvement processes Required Qualifications Current RN License in good standing in the state of practice Bachelor's degree in nursing or associates in nursing with another bachelor's degree Minimum of 4 years of clinical experience, including 2 years in case management Certified Case Manager (CCM) certification or commitment to complete when eligible Geriatric care experience is highly desired
