A company is looking for a Care Navigator. Key Responsibilities Coordinate care internally and externally, ensuring effective communication and information sharing Provide education on medication, lifestyle recommendations, and internal protocols to patients and caregivers Assist patients and caregivers with referrals, appointment scheduling, and transportation arrangements Qualifications 5+ years of clinical care navigator experience 3+ years of experience working with patients and caregivers with cognitive decline An unrestricted multi-state compact RN license; additional Massachusetts license is a plus Desire to work in an early startup environment In-depth knowledge of the health plan/payor ecosystem, particularly Medicare and Medicaid
