Key Responsibilities Assess, plan, implement, and coordinate holistic care management for long-term care members Conduct screenings and risk assessments to identify member needs Develop and update care/service plans based on individual needs and clinical data Required Qualifications Active, unrestricted Registered Nurse (RN) license in the state of Texas Associate's or Bachelor's degree in Nursing Minimum 1-2 years of experience in Case Management or Home Health Experience with Managed Care Organizations (MCO) is preferred Must pass a background check
