A company is looking for a Case Manager RN (Remote). Key Responsibilities Assess, plan, implement, and coordinate case management activities to evaluate members' medical needs Develop proactive care plans to enhance short and long-term outcomes for members Conduct assessments and utilize clinical judgment to address complex health and social indicators impacting care planning Required Qualifications Active and unrestricted RN licensure in Mississippi or compact licensure in state of residence 3+ years of clinical experience 2+ years of case management, discharge planning, and/or home healthcare coordination experience Associate's Degree in Nursing (REQUIRED) Certification as a Case Manager (PREFERRED)
