Multistate Licensed RN Care Manager

A company is looking for an RN Care Manager with a multistate license for a remote position. Key Responsibilities Collaborate with an inter-disciplinary care team to develop and update individualized care plans Engage members through assessments and facilitate communication among care teams to address health needs Educate members about treatment options and coordinate with community resources to enhance care delivery Required Qualifications, Training, and Education Nursing degree from an accredited program or a Bachelor's degree in a health care field, or equivalent experience A minimum of three years of experience in nursing, social work, counseling, or related healthcare fields Preferred experience in Medicaid and/or Medicare managed care Current unrestricted clinical license as a Registered Nurse, Social Worker, or Professional Clinical Counselor Case Management Certification is highly preferred
