A company is looking for a Congenital Heart Disease Nurse Case Manager - Remote. Key Responsibilities Make outbound calls to assess members' current health status Identify gaps or barriers in treatment plans and provide patient education Coordinate care for members and make referrals to outside sources Required Qualifications Current, unrestricted RN license in the state of residence 3+ years of nursing experience as an RN 3+ years of Cardiac and/or Pediatric clinical nursing experience in a hospital setting Experience as a Case Manager for an insurance company or within a hospital setting Basic level of experience with Microsoft Word and access to high-speed internet
