Remote LPN Care Navigator

A company is looking for a Care Navigator- LPN/LVN to provide remote patient care and support. Key Responsibilities Manage physiological markers such as blood pressure, weight, and blood glucose Conduct monthly wellness calls and lead collaborative discussions to define health goals Provide preventive health education and manage patient messaging and alerts Required Qualifications Unrestricted LPN/LVN license in a compact state and CT, MA, or NY 1-3 years of clinical experience in patient or disease management Experience with Medicare patients and telephonic care management Ability to work independently and in the EST time zone between 8:00 AM - 5:00 PM Spanish speaking is a plus
