A company is looking for a Care Coach (RN, LPN, LVN). Key Responsibilities Engage and maintain members in active participation in the program and coordinate member care Utilize motivational interviewing and skill-building techniques in communication with members Collaborate with clinical operations and outreach staff to support member enrollment and engagement Required Qualifications Preferred Associate's or Bachelor's degree in Nursing; equivalent education or two years of experience may be considered Extensive experience in a health care consumer-facing environment, performing assessments and implementing care plans Experience using motivational interviewing to facilitate behavior change Extensive experience in a clinical setting working with the Serious Mental Illness population Preferred background and/or interest in mental health and behavioral health
