Georgia Licensed RN Case Manager

A company is looking for a Remote RN Case Manager. Key Responsibilities Assessing the healthcare and psychosocial needs of patients Developing and maintaining individualized care plans in accordance with protocols Providing ongoing outreach and monitoring of patient data to adjust care plans as needed Required Qualifications Licensed Registered Nurse in good standing in the State of Georgia or Compact license 3+ years of RN Case Management experience in an outpatient setting CCM certification preferred, must obtain within 1 year of employment Supervisory experience preferred Ability to meet accreditation and quality standards including NCQA and HEDIS
