A company is looking for a Health Coach - Wellness to assist members in improving their health and wellness. Key Responsibilities Complete health assessments and implement care plans for members with chronic conditions Provide telephonic support and coaching on wellness care plans and programs Monitor wellness plans and recommend adjustments based on member needs Required Qualifications Associate's degree with 2 - 4 years of related experience Registered Dietitian, Physical Therapist, Respiratory Therapist, RN, Master's in Behavioral Health, or Health Coach Certification required Must reside within the Pacific Standard Time or Mountain Time zone Commitment to working at least 2 late days per week and one late Friday per month
