Licensed Telehealth Nurse

A company is looking for a Telehealth Nurse (LPN/LVN) to provide care to patients with chronic conditions through remote monitoring. Key Responsibilities Perform regular telehealth calls to a dedicated panel of members Provide medical, diet, and wellness education to members Maintain accurate documentation and escalate member issues to the Provider team as needed Required Qualifications Current, Active, and Unrestricted LVN/LPN multi-state compact license Minimum of 2 years hands-on direct patient care nursing experience Telehealth/Remote and chronic care management experience is preferred Tech-savvy with experience using MacBooks and multiple computer applications Reliable high-speed internet and ability to work independently
