Louisiana Licensed RN Case Manager

A company is looking for a part-time RN Case Manager (Remote). Key Responsibilities Conduct patient outreach to improve HEDIS measure compliance Coordinate care and educate patients on preventative care and chronic disease management Document patient interactions and assist in developing strategies to enhance HEDIS performance Required Qualifications Licensed Registered Nurse in good standing in the State of Louisiana or Compact license 3+ years of RN Case Management experience Bilingual skills (English, Creole or French) are a plus Familiarity with HEDIS measures and quality improvement initiatives Ability to meet accreditation and quality standards including NCQA and HEDIS
