Key Responsibilities Provide comprehensive medication management services to members primarily telephonically and face to face Document medication management services for care coordination with members and their care teams Participate in cross-functional care team meetings to improve clinical programs and enhance processes Required Qualifications, Training, and Education Doctorate of Pharmacy (PharmD) or equivalent degree Current unrestricted Minnesota licensure to practice pharmacy Graduated from a college of pharmacy after May 1996 or completed an approved ACPE program 3+ years of related professional experience Strong clinical pharmacy knowledge
